A Croatian citizenship paperwork or certificate, which presents the official decision of the Minister of the Interior to grant you a Croatian citizenship, can be used. Moreover, you must present the original copy of your birth certificate. In case you were not born in Croatia, you must show a sealed and certified Croatian translation of your birth certificate. Lastly, provide two 3.5 cm x 4.5 cm photos and your proof of three-month residence.

You must contact your nearest Croatian consulate or embassy. A first-time application for a Croatian passport should be done in person. Then, ask a consular service whether present application forms can be sent to you via mail or not, so you can fill those papers out before your interview.
